Medication given at school:
Any student who is required to take medication must do so in the health office.
Medication must be in its ORIGINAL CONTAINER with the student’s name, the dosage, time, doctor’s name, and the medication and prescription number. Only the amount required for school consumption needs to be brought to school. Standard district forms for parent’s signature and doctor’s order must be filled out and returned to the Health Office. The Parent/Guardian and Physician Request for Medication form must be filled out. These forms are available in the Health Office. PRESCRIPTION MEDICATION WILL NOT BE ACCEPTED IN A PLASTIC “BAGGIE." Medication MUST BE DELIVERED TO THE SCHOOL BY THE PARENT /GUARDIAN OR OTHER RESPONSIBLE ADULT. Medication or antibiotics that are prescribed two times a day usually do not need to be given during the school day.
Any student who needs an inhaler must have Parent/Guardian and Physician Request for Medication form filled out. If the student needs to CARRY AN INHALER on him/her during the school day, the doctor MUST INDICATE THAT IN WRITING on the district form.
Any student who needs to go home during regular school hours due to illness or injury MUST BE RELEASED THROUGH THE HEALTH OFFICE. Parents or adults on the student’s emergency contact list MUST BRING VALID IDENTIFICATION in order to pick up your student.
If your student has acquired any significant health problems since pre-registration that would interfere with his/her attendance or learning process, please notify the health clerk at Ball Junior High in writing so that teachers can be notified and respond accordingly.
